Appointments Predominantly In-Person or Telemed (Option)
We know that your time is valuable because we value time as well. Telehealth gives the option of being seen when it is convenient for you. You will need the ability via cellphone, tablet, laptop or desktop computer with camera and mic for your appointment. Our connections for videos as well, as our electronic health record platform ensure your privacy. Privacy is your right, and we believe in it absolute.
You must be located at time of telehealth appointment in Iowa.
Upon connecting from our website to our secure electronic health record platform with Optimantra and selecting a convenient appointment time for you. You will be sent, upon scheduling, emails and/or texts for intake form completion and consents requiring to be signed to assist in your appointment time being less paperwork processing and more time spent with you.
CLINIC SERVICES:
Minor acute illness (possibly not minor to you) over 2 years old of age:
-Cough/Cold/Sore Throat
-Earaches (however may determine need to be seen in person based on symptoms/evaluation unless you have a digital ear camera otoscope which are readily available and low investment to have in home)
-Eye Infections
-Fevers/Flu
-Sinus/Allergy Symptoms
-Insect Bites
-Rash (contact dermatitis/poison ivy, impetigo, fungal/ring worm/yeast/athlete's foot)
-Bladder/Urinary Infections
*Provider will determine if alternate level of medical care or in person visit is warranted if you are being seen via telemed
Cost $60/visit
In-Office Injection Administration or Phlebotomy (outsider provider orders):
-Medication Injections (not immunizations) $20
-Phlebotomy $20
*Additional labs at discretion of provider are additional fee. We provide phlebotomy/lab draw in our physical clinic by appointment for fee of $20, or you may present to Quests lab for lab draw. We have contracted lab rates with national labs and share the cost savings with you, but you also have option of utilizing your insurance benefits for labs.
